The Certified Nurse Aide (CNA) is responsible for providing care to patients in their homes under the supervision of a Registered Nurse (RN).

Job Duties and Responsibilities

  1. Provide assistance with activities of daily living including but not limited to bathing, toileting, dressing and grooming.

  2. Represent the agency professionally by demonstrating excellence in patient care, attention to infection control, and strong written and verbal communication skills.

  3. Remind clients to take medication at the appropriate time.

  4. Plan and prepare meals according to specific dietary requirements of the client, and if necessary, feeding the client.

  5.  Assist with ambulation with or without mechanical aids. 

  6. Maintaining a safe, clean and healthy environment through light housekeeping

  7. Providing companionship and stimulation for the client including reading, walks, etc.; and accompanying the client to doctors or other appointments.

  8. Taking client’s vital signs

  9. Observing and reporting changes in client's condition to this agency's Administrator and supervising RN

  10. Accurately preparing daily records and submitting them

A Home Health Nurse provides one-on-one care for clients in the home setting, including dressing, bathing, tube feedings, IV therapy, medication management, wound care, and more. Additionally, the RN is responsible for overseeing the CNA and/or HHA assigned to the case. The RN observes patients health status, performs head-to-toe physical assessments, draws labs, assistance with mobility and more.
